Request an appointment or call to talk about what brings you in. Confirm your appointment time with the office. If you use Zocdoc, check the new- or existing-patient selection and your visit reason.
Review the new-patient forms and gather the health and insurance information requested. Ask the office how to return completed forms.
At the appointment.
Your first visit includes an examination and gathering information relevant to your oral health. Tell the dentist about your concerns and ask what the findings mean for your care.
Costs and insurance.
Before treatment, ask for an explanation of the proposed care, its cost and any payment arrangements. Contact the office with your plan details to discuss insurance. A carrier name alone does not establish coverage for a particular treatment.

A downloadable fillable PDF is available. Ask the office whether to bring it to your visit or use another return method.
